誓約書が未提出の方は、下記URLからダウンロードのうえ、手書きで必要事項を記入し、
附属図書館カウンター(２階)に提出してください。
また、宛のメール添付による誓約書の提出も
受け付けています。誓約書はPDF形式での提出をお願いいたしますが、
PDFで提出できない方は、写真撮影したものでも差し支えございません。
